Residents in assisted living facilities often have little access to transportation and rely on family members' schedules to see a healthcare provider. We want to help you avoid re-hospitalization. We can coordinate with your hospital healthcare providers and continue care as you transition from the hospital setting to home. Medication Management
We will individualize care plans and schedules to help patients manage their medications so they take them on time and every time. In order to avoid any incorrect medication administration, we have the unique ability to evaluate dosage and directions on all home medications in their original bottles with prescribed labels. In the event that a patient cannot afford medications, we will connect them to community and pharmaceutical resources to help with the costs or transition to another effective prescription within their price range.Medication Management
